Getting Home is Easy!

Our facility is open for visitation 24 hours a day, 7 days a week. However, in order to provide the respect and privacy that all of our residents deserve, we request that facility visits be planned between the hours of 10 AM and 8PM. If things run a little later, we ask that those visits be moved from the residents apartment to one the several comfortable seating areas on the premises, so that no other residents are disturbed.

Of course, this is our residents’ home and like the home they lived in before moving here, they are free to come and go as they please. We do ask that residents return to the premises by 11PM, or that they notify us if they will be out later, or overnight.

Located within the Triboro Center for Rehabilitation and Healthcare, we are easily reached by car, bus or subway.

By Car – We are located at the intersection of Teller Avenue and 167th Street. From the Grand Concourse, head east on 167th Street for seven blocks, then turn left onto Teller Avenue. A commercial parking garage is located 100 feet past the entrance of the facility, on Teller Avenue.

By Bus – The NYC Transit Authority operates bus service throughout the five boroughs with stops within easy walking distance of our facility. Busses running on Morris Avenue, Webster Avenue, Melrose Avenue and Jerome Avenue all stop within a short one or two block walk of our front door.

By Subway – From the D or B lines stopping at 167th Street, walk East on 167th Street for seven blocks to Teller Avenue and turn. Our front entrance is a few steps up Teller Avenue on your right.
