Centers Health Care Secures AHA & AMA Target: BP Gold Recognition for Year One

[New York, NY] – Centers Health Care, one of the largest and most complete post-acute health care continuums in the northeast and Midwest with nearly 50 skilled nursing and rehabilitation facilities has secured Target: BP Gold Recognition by the American Heart Association (AHA) and the American Medical Association (AMA) in only its first year of participation.  The AHA recognizes Centers Health Care’s commitment to the improvement, leadership and advances of cardiovascular health, thus achieving this very prestigious recognition this year.  

“This award of excellence makes evident that the fruits of Centers Health Care’s efforts to impact the health of Centers’ patients across geographies and cultures are abundant,” said Victor Bory BSc, MSOD, Director of Community Impact at the American Heart Association. 

According to the AHA, Centers Health Care’s high performance that the company achieved is truly exemplary. The company’s data submitted showed an exceptional 70 percent or higher blood pressure control rate throughout the adult patient population across the Centers Health Care family.

The Target: BP Recognition Program celebrates physician practices and health systems, who treat patients with hypertension, for achieving blood pressure control rates at or above 70 percent within the populations they serve. These achievements will ultimately lead to a reduction in the number of Americans who suffer heart attacks and strokes.
