Centers Health Care Officially Takes Over Focus Otsego in Cooperstown

Renamed Cooperstown Center, Facility to Provide the Proven Centers Brand of Post-Acute Care to the Otsego Communuity

BRONX, NY- January 17, 2018 — Centers Health Care , the largest and most complete post-acute health care continuum in the Northeast, with 42 skilled nursing and rehab facilities spanning New York, New Jersey and Rhode Island, has officially announced today that Focus Otsego located at 128 Phoenix Mills Cross Road in Cooperstown, NY, which was part of the Focus Family of Companies , is now members of the Centers Health Care family.  Focus Otsego will now be known as Cooperstown Center for Rehabilitation and Nursing.

Since last April, we’ve consulted with Focus management and leadership to help them introduce our proven levels of care and service. Now that the transition is complete in Cooperstown, our highly respected brand of industry-leading best practices in care, service and customer commitment that defines who we are, is official at these two locations.”  

Additionally, Focus Otsego’s Long-Term Home Health Care Program will also be joining the Centers Health Care family.   

Centers Health Care and the Focus group entered into an agreement nine months ago whereby Centers Health Care would provide consulting services to Focus, designed to bring the operational and management expertise of the Centers organization to the Focus facilities.  That consulting arrangement grew into an agreement to purchase the facilities which expands the Centers presence in the upstate New York region to nineteen skilled nursing and rehabilitation facilities, as well as Managed Long Term Care, Adult Day Health Care, Home Care and Assisted Living.  Centers acquired seven facilities in the central and eastern part of the state from Capital Living in September.
